To enrol your child, please book a visit at the service during office hours

• After the visit, you will receive a link to the enrolment form via email

• With the enrolment form, please submit your child's immunisation statement from the Australian Immunisation Register (AIR), you can access this through your myGov/Medicare account.

• And child's birth certificate for the proof of age.
